Introduction
Delivering better outcomes in chronic care isn’t a claim—it’s a reality we can measure. At Sciometrix, our Remote Patient Monitoring (RPM) program isn't just tracking vitals—it’s actively transforming them. Across a large and diverse population of 5,690 patients, we’ve achieved substantial and sustained improvements in blood pressure, blood glucose, and weight, all key indicators of chronic disease management success.
Backed by real-world, longitudinal data, our outcomes reflect what truly matters: long-term health stability, reduced risk of complications, and improved quality of life.

Blood Pressure Control: Reliable, Scalable, and Sustained
Key Outcomes:
47.24% of patients experienced clinically significant BP reduction within 90 days.
30.8% witnessed BP unchanged (no worsening of condition).
Among those who improved, 93.22% sustained their BP reduction at 180 days.
96.23% of those with stable BP remained unchanged at 180 days, showing low relapse risk.
Clinical Significance:
Why does this matter? Sustained BP control dramatically lowers the risk of heart attack, stroke, kidney failure, and early mortality. According to the American Heart Association, every 10-mmHg reduction in systolic BP reduces the risk of major cardiovascular events by up to 20%.
Subgroup Insights:
Patients with systolic BP >179 mmHg (extreme hypertensives) showed 86.88% improvement—a highly resistant population where most interventions often fail.
Patients with moderate elevations (141–179 mmHg) saw a 42.54% reduction.
Even those with hypotension (SBP <90 mmHg) experienced improvement (44.76%)—highlighting our ability to manage both ends of the BP spectrum.
Age Inclusivity:
Patients aged 66–78 years and those over 78 years responded equally well, with nearly 48% BP improvement in both groups. This breaks the common stereotype that elderly patients are less responsive to digital interventions.

Glucose Outcomes: Empowering Diabetics Through Proactive Monitoring
Blood glucose control is vital to preventing long-term complications like neuropathy, vision loss, kidney disease, and cardiovascular events.
Results:
Among hyperglycemic patients, 51.7% reduced glucose levels within 90 days.
89.83% of those sustained improvements in 180 days.
Hypoglycemic patients saw a 45.09% improvement—especially important since low blood sugar episodes can lead to cognitive decline or fatal arrhythmias.
Glucometer Subset Analysis:
50.21% improved glucose control over 90 days.
Among those, nearly 90% sustained control over 180 days, indicating habit-forming health engagement and effective follow-up care.
Why It Matters:
The American Diabetes Association notes that consistent glucose monitoring and management can reduce diabetes-related complications by up to 60%. Sciometrix’s ability to help patients self-regulate in near real-time rather than waiting for quarterly visits translates to both clinical and economic benefits.
Weight Management: The Overlooked Metric in Chronic Care
While not always the primary focus in chronic care programs, weight loss is a critical secondary outcome, especially in patients with hypertension and diabetes.
Observations:
37.93% of patients lost weight in 90 days.
25% of patients' weight remains unchanged.
Cumulatively, 62.93% experienced either weight reduction or weight unchanged, a vital gain considering weight gain worsens insulin resistance and elevates cardiovascular risk.
Holistic Benefits:
Even a 5% reduction in body weight has been shown to significantly improve insulin sensitivity, lower BP, and reduce lipid levels. These secondary benefits create a positive feedback loop, reinforcing primary outcomes.

What Sets Sciometrix Apart?
1. Evidence-Based, Long-Term Control
Unlike many RPM models focused on short-term trends, Sciometrix tracks patients over 180+ days, demonstrating not just quick wins but sustained clinical value. Our data proves that behavior change, when reinforced with real-time feedback, leads to better adherence and outcomes.
2. Patient-Centric Intervention Models
Sciometrix uses personalized risk profiling, adaptive alerting, and targeted interventions by Registered Nurses (RNs). This ensures that every data point drives meaningful clinical action, creating early intervention opportunities that prevent deterioration and hospitalization.
3. High-Risk Patients Benefit the Most
We don’t just help the moderately ill—we succeed where care is hardest. Patients with dangerously high BP or severely deranged glucose levels improved significantly, thanks to structured escalation protocols, frequent touchpoints, and nurse-led monitoring.
4. Scalable Across Age and Geography
With patients aged 21 to 102 years, and spread across diverse geographic and socio-economic settings, our results prove that digital health can work for everyone.
Conclusion: Better Data, Better Engagement, Better Outcomes
Chronic disease doesn’t take breaks and neither should care. At Sciometrix, we’ve built a care model that aligns patient behavior, clinical oversight, and proactive intervention to deliver results measurable across thousands of lives.
If you're a healthcare provider looking to improve outcomes without increasing your clinical burden, Sciometrix offers a path forward: scalable, evidence-backed, and truly patient-first.
Let’s deliver better care. Together.
